Loaner Policy & Procedures Loaner

• MedSource generally ships systems into the facility to arrive the day before the scheduled case.
• You must return the system within 2 business days of the scheduled surgery. Generally UPS ground is a sufficient method to reach us by the next business day.
• If the set does not arrive on the 2nd working day, you will be assessed a $25 per day, per set, late fee. This fee will be added to any surgery fee or as a separate billing if the system was not used.
• If additional surgeries arise requiring the system longer, please call 314-966-1818 ext. 118 to make arrangements. Arrangements will be made based on availability.
• There is a $215.00 usage fee on all Arthrotek instrument sets.

Shipping & Handling
• $40.00 REGULAR SHIPPING AND HANDLING
• $150.00 EMERGENCY DELIVERIES
**DELIVERIES BY COURIER MORE THAN 65 MILES AWAY, EARLY A.M. AND SATURDAY DELIVERIES MAY INCUR ADDITIONAL FEES DEPENDING ON SIZE AND WEIGHT.

MTP Reamers
• There is a $175.00 usage fee on all MTP Reamers.

Drill and Guide Wire
• All drills (both solid and cannulated) and guide wires in the Acumed instrumentation sets are for single use only. MedSource of St. Louis, Inc. requests that all facilities discard any drill bits and guide wires used in any surgical procedure. All facilities using the Acumed systems will be charged for the use of guide wires and drill bits.
